Report a Boat Crash, Collision, or Casualty
If you are operating a vessel involved in a crash, collision, or other casualty, you must stop your vessel immediately at the scene of the incident. You must also render assistance to injured persons or attempt to minimize any danger caused by the incident unless doing so would create serious danger for your own vessel, crew, and passengers.
You must provide your name, address, and the identifying number of your vessel in writing to anyone injured from the incident and to the owner of any damaged property.
If the incident results in death or injury to a person, the disappearance of a person from a vessel, property damage in excess of $500, or complete loss of a vessel, you must immediately notify LDWF's Law Enforcement Division, the nearest law enforcement agency, or state police.
For any boat crash, collision, or other casualty, you must submit an Incident Report Form to LDWF within 5 days of the incident. Submit the form to:

To obtain a copy of an incident report, submit an Incident Report Request Form to the above address. Your request must include the following information as well as a check or money order for $7.50*:
- Name and address of vessel operator/owner
- Registration number of vessel
- Date of incident
- Location of incident.
*For copies of photos of the incident, please include an additional $10 in the check or money order.
